118 bed skilled nursing facility includes skilled nursing care, rehabilitation services, secure memory care and assisted living all located on our Carbondale campus. Rehab center features 3,000 square foot therapy gym with state-of-the-art equipment and therapy pool. Physical, occupational and speech therapies available in-patient and out-patient. Short-term rehab, long-term care and respite care are offered. Assisted living offers unique and specialized environment with only 28 studio and one-bedroom apartments where residents receive all the personal care and assistance they need while maintaining their independent lifestyles.
