 Skilled Nursing Facility includes skilled nursing care, rehabilitation services and assisted living all located on our Carbondale campus. Rehab center features 3,000 square foot therapy gym with state-of-the-art equipment and therapy pool. Physical, Occupational and Speech therapies available in-patient and out-patient. Short-term rehab, long-term care and respite care are offered. Assisted living offers unique and specialized environment with studio and one-bedroom apartments where residents receive all the personal care and assistance they need while maintaining their independent lifestyles.
